Add or Edit Custom Statements

Overview

While Chronicler already comes with a multitude of prewritten statements, you can also add custom statements for your specific therapeutic needs and use them in your progress notes.

In this article, you’ll learn how to create and edit custom statements, and we’ll also cover adding a scale to a custom statement.

How to create a custom statement

  1. Login to Chronicler.

  2. Click the “Customize” button in the lower left corner of the note builder.

  3. Click the gold plus sign (“+”) icon in the lower left corner, to open the statement editor.

  4. Enter the desired content for the following fields:

    • “Descriptor”: this is the text that you’ll see displayed in list of selectable statements.

    • “Text”: this is the actual text that will be added to the note, when the statement is selected.

    • “Scale”: allows you to add a scale to your custom statement (details on the available scale types are available later in this article).

  5. Click “Create.”

  6. You’re all set! Your custom statement is added to the list of statement options for the current step, and you can click it to add it your note, just like any other statement.

How to edit a custom statement

  1. Click the “Customize” button in the lower left corner of the note builder.

  2. Find your statement in the list.

  3. Move your mouse onto the statement, so that the blue “Edit” button appears (on a smartphone, the “Edit” is visible by default).

  4. Click “Edit” to open the statement editor.

    • To update a statement, make the desired changes, and click “Update.”

    • To remove a statement, click the “Remove” button in the upper right corner, and then confirm the removal.

  5. You’re all set! Chronicler will apply your changes the next time you add that statement to a note.

Chronicler offers several built-in scales to help you accurately document specifiers in your custom statements when needed.

Scale Types

  • Severity.

    • Options: No, Mild, Moderate, Severe.

  • 0-10.

    • Options: 0-10

  • 1-7.

    • Options: 1-7

  • Strength

    • Options: No, Mild, Moderate, Strong.

How to add a scale to a custom statement

  1. Follow the process earlier in this article to create a new custom statement or edit an existing custom statement.

  2. Add the placeholder {scale} into the “Text” field, where you want the scale selection to be placed.

  3. Click “Create” or “Update” to save the changes.

  4. Chronicler will ask you to choose the desired option for that scale (see Scale Types above for details on options).

  5. You’re all set! The statement is added to the note, with the scale option that you selected.

Scale example

Let’s say that I need to document the severity of a client’s aggression.

  1. I’ll add “{scale} aggression” to the “Text” field of my custom statement.

  2. Then, I’ll add the “Severity” scale to the statement.

  3. Finally, I’ll save the changes, and select the “Moderate” severity option.

  4. Chronicler replaces {scale} with my choice, so the final text in my note reads “moderate aggression.”
